CDM High School has gone digital for their Athletic Clearance process. This is school mandated and required for participation in high school sports. Please see the instructions below.
1. Please fill out and upload the following four documents to your homecampus.com account:
NMUSD High School Activities Form – New this year
Physical Evaluation History Form
Physical Examination Form – Needs a medical doctor’s signature or stamp
Copy of your Medical Insurance card.

2. You may now register online https://homecampus.com or sign in to your existing account.
- Select Corona Del Mar High School.
- Complete parent & student info, emergency contact, medical history, digital signatures. (do not skip any info!)
- If you have any questions, click the “need help” button on the website.
- IMPORTANT NOTE* if you log into your existing account, your information will roll over from last year, but please DO NOT ROLL OVER YOUR PHYSICAL FROM LAST YEAR. EACH SCHOOL YEAR REQUIRES A NEW PHYSICAL FOR EACH ATHLETE. ANY PHYSICALS SHOWING LAST SCHOOL YEAR’S DATES WILL BE DELETED & YOUR CLEARANCE WILL BE DENIED! YOU MUST COMPLETE A NEW PHYSICAL FOR THIS SCHOOL YEAR!
